Medical Marijuana – related visits are not covered by insurance.

In order to become a Medical Marijuana patient:

  1. Must have one of the following conditions: chronic pain, cancer, amyotrophic lateral sclerosis (ALS), Parkinson’s disease, multiple sclerosis, AIDS, damage to the nervous tissue of the spinal cord with objective neurological indication of intractable spasticity, epilepsy, inflammatory bowel disease, neuropathy, or Huntington’s disease associated with severe or chronic pain, severe nausea, seizures, or severe or persistent muscle spasms.
  2. Must have medical records indicating the qualifying diagnosis, dated within the last 6 months.
  3. Must be 18 +.
  4. Must be a resident of New York State with a proof of residency such as a driver’s license or passport.

 

Consult NY State Department of Health web site to make sure your condition qualifies. You will have to ask your current doctors to fax or mail us their recent treatment notes and test results related to your condition.

Initial Visit: $250.00. Follow-up: $100.00. Annual renewal: $150. Patients must pay at the time of visit. Insurance not accepted for this Medical Marijuana – related visits.

Please note that on the first visit we charge an additional $20 deposit towards your 3 months follow-up. That brings the follow-up cost down to $80. Therefore, the cost of the initial visit is $250 + $20.

The $20 will not be refunded if you fail to cancel or reschedule your follow up at least 48 hours in advance. 

We may test our patients for certain substances, such as Heroin, Cocaine, Opioids and/or Alcohol. Please note that, based on your test results, Doctor may not feel comfortable issuing you the Medical Marijuana certificate. In such circumstances you will not be refunded. In some cases, we need to send the samples to a laboratory for confirmation (it may cost additional fees). We will try to minimize this expense, where possible.
